You have the right to:

 

1)  be informed about, to consent to, or refuse the recommended treatment.

 

2)  be treated with dignity as an individual, with compassion and respect, with reasonable protection from harm, and with appropriate privacy.

 

Also…Your medical record will be kept confidential unless disclosure is required or permitted by law or you consent to its release.

 

 

You also have Responsibilities!

 

You should display reasonable and responsible behavior.  This will help the PRRC provide the information and activities to help you in your recovery goals.

 

You are expected to:

 

1)  follow all of the medical center’s safety rules and posted signs.

2)  be considerate and respectful of all medical center personnel and other patients.

3)  cooperate with your treatment staff.  If you have questions or disagree with your treatment plan, you are responsible for discussing it with your treatment staff.

4)  try to prevent any injury to yourself, other patients, visitors, and staff members by your own actions and to be responsible for the safekeeping of clothing, money, and

personal possessions you choose to keep with while you are in this facility.

5)  keep all of your scheduled appointments on time and if you have difficulty notify the PRRC staff as soon as possible.

6)  avoid interfering with the treatment of other patients.

7)  assist by alerting the staff when another patient is having any difficulty.

8)  be understanding and patient if you encounter delays.

9)  make sure you understand when your next appointment is.
